In this video, we present a case of a bowel injury that occurred during laparoscopic repair of a recurrent incisional ventral hernia. The patient was a 65-year-old woman who had undergone a previous ventral hernia repair with mesh placement. She presented with a recurrent hernia at the same site. During laparoscopic dissection, the bowel was found to be edematous and friable. There was evidence of obstruction, and the bowel was adherent to the mesh. The bowel was successfully repaired, and the patient made a full recovery.
This case highlights the importance of careful dissection during laparoscopic repair of recurrent incisional hernias. The presence of mesh can make dissection more difficult and can increase the risk of bowel injury. Surgeons should be aware of this risk and take steps to minimize it.
